
Insurance Plans
Medical and vision exam services are provided and billed by Dr. Jaclynne Magno-Choi. Our office participates in most all of the major vision insurance plans in the region. Below are the health plans which we currently accept.
If we do not accept your insurance plan, we will be unable to file a claim on your behalf. However, we can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.
Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

Our office accepts Visa, Mastercard, Discover and American Express Cards. We also accept payments in cash. We do NOT accept Wal-Mart cards without any Visa/Mastercard logo. Full payments are required at the time of service. NO PERSONAL/BUSINESS CHECKS ACCEPTED.
Payment Methods
​​How do I find out what vision insurance I have?​
There are several ways for you to find out what insurance you may have. First, you may contact your health insurance directly. Their contact information is often located on your health card. Second, you may ask your Human Resources department in order to find out if you have a separate vision insurance along with your health care insurance. Third, contact our office and we may be able to help you.

​How much does my insurance cover for exam?
​​​Your insurance benefits may vary. We will notify you of your examination benefits before we render any services. We will need to obtain an authorization from your insurance before any services can be done.
How much does my insurance cover for my glasses/contacts?
Our office provides examination services only. Therefore, we may be unable to determine your coverage for materials (glasses/contacts). The best way to find out your coverage is contacting your insurance directly or by going to the vision center next to our office.

​​Will I need to pay any out of pocket costs?
​​If your insurance covers only a portion of the examination fee, you are responsible for the copayment along with the remaining balance not covered by your insurance. Most insurances only cover a portion of a contact lens fitting evaluation. Any balance is due at the time the services are rendered.

How do I bill my insurance?
​With most insurances, our staff will handle the billing to the insurance company. Any payments made by your insurance will be addressed and mailed directly to Dr. Jaclynne Magno-Choi. If for any reason, we do not receive a payment from your insurance regarding the services we provided, you are responsible for payment for these services. A remaining balance will be posted on your account. You will be notified by phone, email or by mail.

How do I pay my balance?
You can make a payment with a credit/debit card or cash by visiting OR calling our office.

Why am I paying extra for contact lens fitting when I already wear contacts?
In order to purchase and order contact lenses (CL), a CL exam, a fitting, as it is called, is required by law and needs to renewed yearly upon doctor's discretion. This is an additional set of procedures, separate from a glasses exam, done to assess the vision and fit of contact lenses. CL's are FDA approved medical devices (not cosmetic products) and therefore strictly controlled for their safety for consumer use. However, proper patient wear can vary with each consumer. Therefore, a health assessment is needed YEARLY to verify the continued safety of contact lens wear for the user. The purchase, and the sale of contact lenses WITHOUT a valid prescription is ILLEGAL. And the consumer is putting their eyes and vision at risk without proper evaluations. Most insurances do not have benefits for contact lens fitting as this is considered an upgrade/optional benefit. We can check your insurance for any coverages.
